Inquiring About Our School

You can submit an inquiry to let us know that you are interested in attending our school. Enter ALL your information before you submit your inquiry. You will not be able to review or update your information once you submit your inquiry.

1.	Select the Home tab.

2.	Select the Inquire menu item.

3.	View the Inquire page. Depending on our current settings, the Inquiry form will either be:

•	Displayed on one page. Enter all your information on the page.

•	Displayed over a series of pages. Enter all your information on a page and then select Next to display the next page of the inquiry form.

4.	Enter your Personal Information.


Field:	Description:
Prefix	Specify your starting form of address (for example, Mr., Miss, Ms.).
First Name	Enter your legal first name; not your nickname.
Middle Name	Enter your middle name, if you have one.
Last Name	Enter your last name or surname.
Suffix	If applicable, select your suffix (for example Jr., Sr.).
Nickname	If you have a nickname that you prefer to use instead of your legal first name, enter it.
Former Last Name	Your previous last name, if you were known by a different Last Name (for example, a maiden name).

5.	Enter your Address information.


Field:	Description:
Address Type	A description of the address that you want us to use to contact you (for example, home address, parent’s address, local address). Then enter all the information for this address.
Address Line 1	Enter the first line of your street address.
Address Line 2	If needed, enter the second line of your street address.
Address Line 3	If needed, enter the third line of your street address.
City	Enter the name of your city.
State	Select the name of your state or province.
County	Select the name of your county.
Postal Code	Enter your postal or zip code.
Country	Select the country in which this address is located.

6.	Enter your Contact Information.


Field:	Description:
E-mail Address	Enter the e-mail address that you want us to use to contact you.
Daytime Phone Number	Enter the phone number that you want us to use to contact you between 9am and 5pm. The phone number must be all numbers; do not enter dashes or any other special characters.
Evening Phone Number	Enter the phone number that you want us to use to contact you after 5pm. The phone number must be all numbers; do not enter dashes or any other special characters.

7.	Enter your Academic information.

1.	Choose Select Period.

2.	Select the Year and then, if you want to, you can choose the Term or the Session.

Select the Program in which you are interested and whether you want to attend as a full-time or part-time student.

If you are interested in any other program for the same period, select Add Program and then choose the Program and the Full-time/Part-Time Status.

8.	Select all the academic Interests you would like more information about. If this is a required field, you must select at least one interest.

9.	Enter your Demographic information.


Field:	Description:
Date of Birth	In the format MM/DD/YYYY, enter the date on which you were born. For example, you would enter 07/15/1989 if you were born on July 15, 1989.
Gender	Specify whether you are a male or female.
Ethnicity	Select your ethnic background.
Marital Status	Select your current marital status.
Religion	Select your religion.
Veteran Status	Select your current veteran status.
Retired?	If you are a retiree, check this box.

10.	Enter your Citizenship information.


Field:	Description:
Primary Citizenship	Select the country of your primary citizenship.
Secondary Citizenship	If you are a citizen of another country, select it.
Visa	Select the type of Visa you have.
Country of Birth	Select the country in which you were born.
Primary Language	Select your first language.
Secondary Language	Select your second language.
Months in Country	If you are not a citizen of the country in which our school is located, enter the number of months that you have been in this country.

11.	Enter your Government information.


Field:	Description:
Government ID	Enter the 9-digit identification number that has been assigned to you by the government (for example, your Social Security Number). Enter your ID number without dashes.
SEVIS Number	If you are not a citizen of this country, enter your Student & Exchange Visitor Information System (SEVIS) number.
Driver’s License Number	Enter your driver’s license number.
Driver’s License State	Select the state which issued your driver’s license.
Visa Number	If you are not a citizen of this country, enter the number of your Visa.
Visa Country Issued	Select the country which issued your Visa.
Visa Expiration Date	Enter the date on which your Visa will expire.
Passport Number	If you are not a citizen of this country, enter your passport number.
Passport Country Issued	Select the country which issued your passport.
Passport Expiration Date	Enter the date on which your passport will expire.
Port of Entry	Select the port from which you entered this country.
Date of Entry	Select the date on which you entered this country.
Criminal Convictions?	If you have been convicted of any crimes, check this box.

12.	Check each of the Sources which provided you information about our school. If this is a required field, you must select at least one source.

13.	Review your information.

14.	If all the information is correct, select Save to submit your inquiry.
